If your baby always looks over the same shoulder, the most common reason is a tight neck muscle, a condition called congenital muscular torticollis. It is common, and it usually responds well to simple care. The earlier it is noticed, the easier it tends to be to correct.
Many parents first spot it in the early weeks, when the head keeps resting tilted toward one side in the car seat or the crib.
What torticollis is
Torticollis means “twisted neck.” In babies, it usually involves the sternocleidomastoid, a muscle that runs along each side of the neck. When that muscle is short or tight on one side, the head tilts toward that side and the chin turns the other way. That is how the American Academy of Pediatrics’ guide to head tilt in children describes it.
The AAP describes congenital muscular torticollis as the most common cause of head tilt in children under five. It is usually related to how the baby was positioned in the womb, and less often to a difficult delivery. Pediatricians typically notice it in the first six to eight weeks after birth.
Early signs parents can spot
- The head tilts toward one shoulder most of the time, including during sleep
- Your baby turns easily to look one way but struggles or fusses when turning the other
- A strong preference for one breast, or one side during bottle feeding
- A flat spot developing on one side or the back of the head
- A small, soft lump in the side of the neck, which the AAP says can be felt in about half of cases
A preference for one side does not always mean torticollis. A quick check with your pediatrician is the simplest way to know.
Why it helps to catch it early
Cleveland Clinic notes that torticollis often improves when treated early, ideally within the first month or two. In its article for parents on correcting a baby’s head tilt, it adds that treatment can take longer after about 3 months of age.
Timing also matters for head shape. When a baby keeps resting on the same spot, the head can flatten on that side. The AAP notes that untreated torticollis may lead to lasting facial unevenness and limited head movement.
Because torticollis can occur alongside a hip condition called developmental dysplasia of the hip, your pediatrician may check your baby’s hips as well.
What usually helps
For most babies, care is simple and done at home, guided by your pediatrician or a pediatric physical therapist. The AAP and Cleveland Clinic both describe:
- Gentle stretches several times a day, done exactly as your baby’s provider shows you, and never forced
- Placing toys, faces and activity on the side your baby tends to avoid, so turning that way becomes the interesting direction
- Supervised tummy time while your baby is awake, which builds neck strength and takes pressure off the back of the head
- Changing which end of the crib your baby faces, while always placing them on their back to sleep
The AAP says these simple steps correct the problem in the vast majority of babies. Physical therapy is often recommended when the tilt is significant or is not improving with home care.
Where chiropractic fits
Torticollis should be evaluated by your pediatrician first, and stretching and physical therapy are the standard care. Some families also ask about chiropractic. The research on chiropractic for infant torticollis is limited, so we do not promise results.
What we can offer, alongside your pediatrician, is a gentle evaluation of how your baby moves, turns and feeds, with very light contact adapted to a newborn. Our page on torticollis care in Colleyville explains how chiropractic fits alongside your pediatrician and physical therapist, and pediatric chiropractic care at Radiant covers how pressure and positioning change for babies. If you are still deciding whether chiropractic is right for your child, the questions parents ask before a first visit cover safety, comfort and what happens at the first appointment.
Call your pediatrician first if…
A head tilt should always be checked by your baby’s doctor. Get in touch promptly if:
- The tilt appeared suddenly, or your baby seems to be in pain
- Your baby has a fever, is unusually sleepy or irritable, or is not feeding well
- You notice crossed eyes, unusual eye movements or concerns about hearing
- The tilt is not improving with the stretches and positioning your provider recommended

Dr. Cameron is an amazing chiropractor who focuses on the issues and creates a plan to adjust the issues to sustainable living. Went from migraine/tension headaches daily to having nothing for past 2 weeks. I had an accident that put me out of alignment, yet Dr. Cameron has taken the time to assess and progress with each area and issue. Has alleviated numbness in my fingers from my wrist issue and recovering a majority of the lost flexibility in my neck. Posture is improving and the tension I carried between my shoulder blades to the base of my neck is relieved.Posted on Google![]()
